Gum Boil? See What’s Cooking
A gum boil can appear very quickly and with no prior warning. I’ve treated patients who told me that they went to bed wit no problem and woke up to find a small localized swelling and a painful tooth. When this happens most people have the some questions:
1. What is this small localized swelling? It is simply a pocket of pus trapped between your gum and the underlying bone.
2. What are the causes? There are 2 common types. The first is caused by a gingival abscess. In this type you will get a pocket between the tooth and gum. This pocket becomes infected and the pus builds up under the gum. Th second type comes from a root canal infection. This means that the dental pulp (the soft tissue on the inside of the tooth) gets infected and drains to the area at the tip of your tooth’s root. When this is not treated immediately with a root canal procedure the infection spreads and causes a pocket of pus to form.
3. Are they painful? When onel first forms it can be tender because of the pressure building beneath the gum. Eventually the building pressure will puncture the overlying gum tissue and form a small hole for the pus to drain. When this happens we refer to it as a draining sinus tract. Once it drains through this small hole there is usually no pain.
4. How are they treated? If it is caused by a gingival abscess the pocket will need to be cleaned. Sometimes a periodontal surgical procedure will be necessary. If it caused by a root canal infection that means you must have the infected dental pulp removed. There are two options that accomplish this. The first is tooth removal. The second is endodontic treatment. The root canal procedure involves making a small opening in the tooth. Small endodontic instruments are then used to clean the infected pulp from inside the roots. The space left from this cleaning process is then filled with material such as gutta percha or Resilon.
5. If I have a root canal won’t it hurt? Endodontic therapy has a very bad reputation for causing pain. The reality is that most are performed very comfortably. It would be a mistake to treat your gum boil with tooth removal because of fear of an endodontic procedure. If at all possible I would strongly encourage you to save your root with a root canal procedure.
